Understanding Motor Neurone Disease

What is Motor Neurone Disease and How Does It Progress?

Motor neurone disease (MND) is a group of progressive neurological conditions that affect the motor neurones in the brain and spinal cord. These are the nerve cells responsible for controlling voluntary muscle activity, including walking, speaking, swallowing, and breathing. As the motor neurones deteriorate, the muscles they control become progressively weaker. There is currently no cure for MND, but specialist care at home can make a profound difference to quality of life, independence, and the time families spend together.

MND progresses differently in every person. The most common form, amyotrophic lateral sclerosis (ALS), typically affects both upper and lower motor neurones. Progressive bulbar palsy (PBP) affects speech and swallowing more prominently at onset. Progressive muscular atrophy (PMA) begins with lower motor neurone weakness. Each has a different trajectory and a different pattern of clinical need. Our carers are trained to understand these distinctions and respond appropriately as each stage unfolds.

At SD Care, we work closely with neurologists, MND specialist nurses, physiotherapists, speech and language therapists, and respiratory teams to build a care plan that genuinely keeps pace with change. Our complex care team handles clinical tasks including ventilator management and PEG feeding, and our live-in care team provides round-the-clock presence when that becomes necessary.

  • Personal care adapted to changing physical ability as MND progresses, delivered with full dignity
  • Repositioning, pressure area monitoring, and safe moving and handling throughout the day and night
  • Communication support using augmentative and alternative communication (AAC) devices and eye-gaze technology
  • Assistance with eating and drinking, including modified texture diets and safe swallowing techniques
  • PEG feeding management where clinically indicated and in line with the dietetic team’s guidance
  • Non-invasive ventilator support including BiPAP and CPAP management by competency-verified carers
  • Medication management including subcutaneous routes where the carer holds appropriate competency

Signs to Look Out For

When Should You Consider
Professional MND Care at Home?

Knowing when to bring in professional support is not always straightforward. These are the situations families most commonly describe when they first contact us about motor neurone disease.

Increasing personal care needs

Muscle weakness has progressed to the point where washing, dressing, and personal care can no longer be managed safely without a trained professional present.

Swallowing or nutrition difficulties

Your loved one is experiencing difficulty eating or drinking safely, has lost weight significantly, or requires a modified diet or PEG feeding that needs skilled, consistent management.

Communication is becoming affected

Speech has changed significantly and your loved one uses or is transitioning to an AAC device, letter board, or eye-gaze system. Carers need to be familiar and confident with these tools.

Respiratory support is required

Your loved one has been prescribed non-invasive ventilation and requires a carer with specific, verified competencies to manage respiratory support safely at home.

Family carers are reaching their limit

Family members have been the primary carers and are physically and emotionally depleted. Professional support gives everyone permission to be present as family rather than as carers.

Planning ahead after diagnosis

Your loved one has recently been diagnosed with MND and wants to plan ahead so the right specialist team is in place before needs become more complex and urgent.

Overnight support is now needed

Your loved one requires regular assistance through the night, perhaps with repositioning or respiratory equipment, and nobody in the household is getting adequate rest.

A clear wish to remain at home

Your loved one has expressed a firm wish to spend their remaining time at home. The right specialist care team makes this not only possible but well-supported.

  • ISO 9001:2015 Certified by DNV

Our quality management is certified to ISO 9001:2015 by DNV, providing independent assurance that our carer selection, competency verification, care plan review, and clinical liaison processes are rigorous.

  • Specific MND Clinical Training

Our carers receive training that goes beyond general care skills, covering MND progression, communication support, safe moving and handling for weakened muscles, and respiratory care.

  • Consistent Carers Who Know Your Loved One

We assign a small, stable team to each MND client and keep disruption to an absolute minimum. The carer who learns your loved one’s communication patterns early is the same person providing support as the condition evolves.

Frequently Asked Questions
About MND Care at Home

Navigating an MND diagnosis raises many questions about care, funding, and what is actually possible at home. Here are honest answers to the ones families ask us most often.

What is motor neurone disease?

Motor neurone disease (MND) is a group of progressive neurological conditions that affect the nerve cells controlling voluntary muscle movement. As these neurones deteriorate, the muscles they control gradually weaken. MND affects movement, speech, swallowing, and eventually breathing. There is currently no cure, but specialist care at home can significantly improve quality of life.

The main forms are amyotrophic lateral sclerosis (ALS), progressive bulbar palsy (PBP), progressive muscular atrophy (PMA), and primary lateral sclerosis (PLS). Our carers are trained to understand and respond to the distinct needs of each form.

Can someone with MND be cared for at home?

Yes, and for the majority of people living with MND, remaining at home with the right specialist team in place is entirely possible. With a well-constructed care package, the complex clinical and personal care needs associated with MND can be managed safely and compassionately at home.

Our care adapts as the condition progresses, so your loved one is supported at every stage without unnecessary disruption. Learn more about live-in care for MND.

How does MND care differ from standard home care?

MND care requires carers with specific training in neurological conditions and, as the condition progresses, formal competency verification for clinical tasks. This includes understanding MND’s different forms, working safely with communication aids, managing ventilation equipment, supporting PEG feeding, and recognising respiratory deterioration.

Our complex care team specifically provides this level of clinical specialist support.

Do your carers have experience with AAC devices and communication aids?

Yes. As MND progresses, speech is often significantly affected, and many people use augmentative and alternative communication (AAC) devices, letter boards, or eye-gaze technology. Our carers receive specific training in how to work confidently and sensitively alongside these tools, ensuring your loved one’s voice and choices remain central throughout the progression of the condition.

Can you support someone with MND who needs ventilation at home?

Yes. We have carers with specific, documented competency in non-invasive ventilation (NIV) support, including BiPAP and CPAP equipment. Competency is formally verified against the specific equipment prescribed before care begins. We work in close liaison with the respiratory team and community nursing service throughout.

What is NHS Continuing Healthcare and is someone with MND likely to be eligible?

NHS Continuing Healthcare (CHC) is a package of care funded entirely by the NHS for people whose primary need is a health need. People living with MND frequently meet the eligibility criteria, particularly as the condition progresses and clinical needs become more complex. If eligible, the NHS funds the full cost of the care package at home.

We support families through the CHC assessment process and, where appropriate, through appeals. Learn more about NHS Continuing Healthcare and other funding options.

How quickly can MND care at home be arranged?

We understand that MND care is often needed urgently. In urgent situations we act as quickly as possible. For planned care, we encourage you to contact us as early as possible following diagnosis so the right foundations can be put in place before needs become more complex.

Please call us and we will talk through the options at whatever pace suits your family. There is no obligation and no pressure.
